I had a patient whose wife kept saying would get staggering-drunk, then go out and get in his semi-tractor and drive. It was a huge ordeal to get anything done to check him out, confirm it was true, and intervene. Thankfully nobody was killed in the interim.

On the other hand the CDL exams got so nit-picky that my partner, who was certified to do PILOT physicals, quit doing CDL ones, because the bureaucratic part got so ridiculous.
