Paco's daughter just sent me a message saying "We just got followup xrays and his pneumonia is worse. They are sending us to the hospital so he can get IV antibiotics and possibly have fluid drained."

Additional prayers from my house. When I had Mycoplasma pneumonia 20 years ago, the IV antibiotics worked their magic, even if the hospital bed tried to kill me.

Darnedest thing with that pneumonia. I was out mowing the lawn and could hardly push the mower. Killed the engine and even checked the wheels to see if something was binding them. Lawnmower sure seemed heavy. Then I realized I was feeling oddly weak. Went in, took my temp and it was 104. ...

He might need a combination of antibiotics. Usually they give one at a time. Problem with that is some bacteria can become resistant at practically light speed. A 1-2 or a 1-2-3 punch of totally different antibiotics overwhelms the bacteria's ability to change reading frame on it's DNA codons and it usually becomes extremely susceptible to one antibiotic while trying to avoid the other 1 or 2.

Piller's right - only when I have a patient with bad pneumonia I'll use Augmentin 500 TID with Keflex 500 TID and Zithromax 500 daily for 5 days. They are synergistic, with the beta-lactamase producers rendered vulnerable to the cephalosporin due to the clavulanic acid, and any of the little bacterial ***'s that do manage to make it to division after ribosomal sabotage via the azithromycin will explode when their cell-walls are sabotaged from the amoxicillin and cephalexin. Carpet-bomb the infection that way for five days and the immune system has time to send in the troops and clean up the mess.

Sometimes substitute Cipro for the Zithromax, but be prepared for tendon or aorta issues.

Do all the airway and cardiac support stuff you can in the interim.

Prayers up Paco kicks butt and the docs help win the battle. Show him pictures of his wife and kids and grandkids and he will likely get better so he can still be with them.
