I could tell that I was sick as soon as I got it. Spreaders who do not know they have it seem rather unlikely to me.

Everyone reacts to viruses differently. Some get it, manufacture antibodies and never know it. Some die.
I happen to react very badly to flu. Heck even with the yearly vaccines, it may well kill me. My wife rarely gets ill. Avoidance and immunization are of more or less effective from individual to individual.
Knowing what I know about myself, I choose to do what I can to remain well.

My wife got it and was slightly feverish for 27 hours. She just shrugged it off, but she knew she had it. She didn't feel quite right.
